OSWEGO 鈥?The Oswego County Health Department has announced it will stanley cup  no longer admit new patients to the Oswego County Long Term Home Health Care Program.The County Health Department is following New York State guidelines and working closely with local health care providers and contractors since receiving approval from the NYS Department of Health to close the Oswego LTHHCP.Four hundred fifty letters were sent to providers and agencies.All of the Oswego LTHHCP patients have been transferred to managed long term care agencies or nursing home transition and diversion Medicaid waiver programs.The county Health Department will continue to follow New York State guidelines in the process of closing the county Long Term Home Health Care Program.Although the Oswego LTCHHP is closing, the Hospice and Certified Home Health Care programs continue to serve Oswego County residents.
